Despite extensive investigations, there is currently no evidence of identity theft or fraud resulting from this incident. Pacific Cataract and Laser takes the security and privacy of information seriously, and has taken immediate steps to address the situation.
The incident was identified on November 13, 2023, when suspicious activity was detected within the network. Pacific Cataract and Laser promptly secured the network and launched an investigation to determine the nature and extent of the breach. It was revealed that an unauthorized actor had access to certain systems containing files with personal and protected health information between November 13 and 14, 2023. While Pacific Cataract and Laser has not found any evidence of identity theft or fraud, individuals whose information may have been affected are being notified.
The Institute has notified federal law enforcement and the U.S. Department of Health & Human Services’ Office for Civil Rights, and is fully cooperating with all investigations. The compromised information may include names, medical treatment details, health insurance and claims data, financial account information, driver’s license numbers, Social Security numbers, and demographic information like birthdates. Pacific Cataract and Laser advises affected individuals to monitor their accounts for any suspicious activity, review their credit reports, and keep an eye on explanation of benefits forms.
Additionally, the Institute is providing contact information for the three major credit reporting agencies – Equifax, Experian, and TransUnion – and recommendations on obtaining free credit reports and placing fraud alerts on credit files. Individuals can also find resources on identity theft, fraud alerts, security freezes, and steps to protect their information from the credit bureaus, the Federal Trade Commission, or their state’s Attorney General.
